I understand my testimonial statement as outlined above (the "Testimonial"), my name and likeness made on behalf of West Virginia Central Federal Credit Union may be used in connection with publicizing and promoting the credit union or for any other legitimate purpose. I authorize West Virginia Central  Federal Credit Union to use my name, brief biographical information if necessary, my likeness and the Testimonial as set forth on this form.

I hereby irrevocably authorize West Virginia Central  Federal Credit Union to copy, exhibit, publish, use or distribute the Testimonial for purposes of publicizing West Virginia Central Federal Credit Union  or for any other lawful purpose. The Testimonial, my name and likeness, may be used in printed publications, multimedia presentations, on websites or in any other form. I agree that I will make no monetary or other claim against West Virginia Central Federal Credit Union for the use authorized above.

In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my name, likeness or my Testimonial appears.

I hereby hold harmless and release West Virginia Central  Federal Credit Union from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of the use authorized above.

I have read and understand the authorization and release information and give my consent for the use as indicated above.
